System, Photo ID, Memorandum from your department chair or campus
personnel officer, signed and dated on official
letterhead, stating that you have worked 10 or more years in the UH
system

Borrowing Privileges: 10 items
Renewal of Items: 1 time only
Replacement Fee: $5 per card
Card Issued: Immediately upon completion of required forms
